Elder Advocates Incorporated is an independent care management company dedicated to assisting clients and their families in researching and acquiring needed care management services. Elder Advocates Incorporated was founded in 1998 by its owner, Patrice Antony. Patrice is a Physical Therapist and Geriatric Clinical Specialist with over 27 years of experience in working with the handicapped and the elderly. She formed Elder Advocates Incorporated after watching too many families struggle in crisis when an elderly parent or loved one became ill. The stress and strain of managing crises was overwhelming for many caregivers - especially those who lived miles away and had families and jobs to manage as well. The need to hire a professional to manage the care for a loved one - both in and out of the rehabilitation or hospital setting was apparent.

The goal of Elder Advocates Incorporated is to assist people in getting the objective information they need to make good decisions in acquiring necessary services and eligible benefits. We visit the facilities and/or services that we recommend so that we have first hand knowledge of what they have to offer. We perform comprehensive assessments of each client's needs so that we can meet those needs with the most appropriate services. This assessment includes:

• medication review
• functional assessment to identify needs
• safety assessment
• financial analysis so that recommendations are in line with your budget
• benefit eligibility analysis
• support system analysis
• referrals to appropriate services

Elder Advocates Incorporated refers clients to other professional services that may include: Elder Law Attorneys, Pharmacists, Rehabilitation Specialists, Dieticians, Physicians, and many others. Since we are an independent company, we do not share financial arrangements or referral fees with any of these entities, so you can be sure that you are getting an objective recommendation.
